License of adult home in Olympia taken by state

The state Department of Social and Health Services has halted new admissions and revoked the license of Lakeside Gardens AFH LLC, a privately operated adult family home located at 3441 14th Ave. in Olympia.

The adult family home is licensed to Jennie Carkhuff.

The state took the action after finding numerous violations that jeopardized the safety and well-being of residents. The violations include but are not limited to:

 • The provider failed to ensure a resident received necessary care and services to prevent repeated falls.

 • The provider failed to request emergency assistance after a resident sustained facial injuries in a fall.

While the home is in stop placement, the licensee may not admit residents.

The provider has the right to contest the stop placement and revocation by requesting an administrative hearing within 28 days of state notification. Current residents may stay in the home. If any residents choose to remain, DSHS will continue to monitor the home for resident safety.
