The Washington State Department of Health revoked or suspended the licenses, certification or registrations of two health care providers in Thurston County in September.

The department suspended for at least two years the certified nursing assistant credential of Amber Lee Cotten. Cotten took about $30 from a patient at the assisted living facility where she worked.

The department suspended for at least seven years the certified nursing assistant credential of Norma L. Azurin. Azurin was convicted in Thurston County Superior Court of four counts of first-degree theft, all Class B felonies, and one count of obtaining a signature by deception or duress, a Class C felony.
