The state Department of Social and Health Services has stopped new admissions and revoked the license of an adult family home in Lacey after finding that regulations were violated that affect the safety and well-being of residents, a press release said today.
The Colonial House Adult Family Home Care is located at 4810 18th Avenue.
Violations include but are not limited to the licensee's failure to:
• Maintain a safe medication system and meet the physical and behavioral needs of three residents.
• Obtain a written assessment and preliminary service plan for one resident and failure to read assessments for two residents to ensure their needs could be met before moving into the home.
• Meet one resident’s behavioral care needs.
• Ensure one resident received his medication as directed by the physician.
• Ensure an accurate medical log was kept for one resident.
The provider has the right to contest the state’s actions by requesting an administrative hearing within 28 days of state notification. Residents may stay in the home pending resolution of the appeal process, but new admissions or re-admissions are prohibited without DSHS permission.
DSHS will continue to monitor residents’ safety if they choose to remain there.
