The state Department of Health has suspended the credentials for two health care providers in Thurston County.
The Secretary of Health Office has suspended the registered nursing assistant credential of William M. Conley for at least 18 months. The action was taken after Conley yelled at and grabbed a vulnerable resident of a group home.
The office also suspended the registered nursing assistant credential of Teresa Sue Congdon for at least one year after Congdon falsified patient records and didn’t follow a patient’s treatment plan at a nursing facility where she worked.
