The state Department of Social and Health Services has stopped admissions to Garden Courte Alzheimer Community, a boarding home at 626 Lilly Road N.E., Olympia.

The boarding home is licensed to Olympia Special Care LLC.

DSHS reported finding numerous violations that jeopardized the safety and well-being of residents. Among them, according to DSHS:

 • The facility failed to ensure that negotiated service agreements were updated and reflected the current care and services for five residents.

 • The facility failed to ensure that enough staff members were available to meet the residents’ needs as outlined in the negotiated service agreements. Residents were awakened, sat in front of the television for extended periods, sat in urine-soaked clothes and were restrained.
