Restaurant inspections for Feb. 9

Comments are taken directly from the latest Thurston County inspection reports, which are available at each food establishment, at the Thurston County Environmental Health Office and their website at co.thurston.wa.us/health/ehfood. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the restaurant has a history of red violations or repeat violations.
  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Jan. 25: 0 red; 8 blue.

Comments: They used an in-adequate, non-commercial refrigerator to store open TCS foods. The bathroom lacked a sanitary napkin receptacle.

Jersey Mike’s Subs

1401 Marvin Road NE, Suite 102, Lacey

Jan. 26: 10 red; 7 blue.

Comments: A slicer was not properly cleaned. The back room lacked ceiling tiles. The person in-charge wasn’t knowledgeable about proper cooking temperatures for chicken. An on-shift worker did not have their food worker card upon request.

Little Caesars

704 Trosper Road SW, Tumwater

Jan. 26: 25 red; 0 blue.

Comments: Pizza timers were not set for pre-made uncooked pizzas at room temperature. The inspector observed excellent handwashing by two on-shift workers.

Plaza Jalisco

5212 Capitol Blvd. SE, Tumwater

Jan. 26: 50 red; 8 blue.

Comments: There was no clear person in-charge on duty. An employee designated as cook and server did not wash their hands after returning to the kitchen. The hand sink near the kitchen lacked a hand drying provision. Chicken and eggs were stored over ready-to-eat vegetables. Chicken in the hot holding container was above the container. In-use ice scoops were improperly stored in room temperature water. Chip containers and the ice machine were not cleaned to sight and touch per food code.

Axis Mead LLC

2633 Willamette Drive NE, Lacey

Jan. 29: 5 red; 5 blue.

Comments: A worker lacked a food worker card. The facility lacked thermal dish machine test strips.

Aya Sushi Restaurant

1540 Cooper Point Road SW, Olympia

Jan. 31: 25 red; 3 blue.

Comments: Chicken at the sushi bar was improperly cooled using ice. In-use utensils were not properly stored either below 41 degrees, above 135 degrees or cleaned and placed on a food contact surface. The inspector noted there was excellent use of time as a control for rice.

Grabajava Espresso

9121 Pacific Ave. SE, Lacey

Feb. 1: 25 red; 12 blue.

Comments: Improper cold holding temperatures. No accurate thermometer provided for potentially hazardous foods. Improper compliance with risk control plan, variance, plan of operation. Warewashing and physical facilities were improperly maintained. Plumbing was improperly maintained too.

Canna Cabana Subs and Shakes

193 Marvin Road SE, Lacey

Feb. 1: 5 red; 0 blue.

Comments: The inspector noted there were expired food worker cards.

Eagan’s Westside Drive-In

1420 Harrison Ave. NW, Olympia

Feb. 1: 25 red; 0 blue.

Comments: The inspector observed no handwashing between tasks for multiple staff.

Top Rung Brewing Company

8343 Hogum Bay Lane NE, Lacey

Feb. 1: 15 red; 0 blue.

Comments: Foods in cold holding were above 41 degrees rather than below as required. An accurate thermometer was not used to monitor food temperatures.

4530 Martin Way E., Olympia

Feb. 1: 35 red; 0 blue.

Comments: Employees did not wash their hands properly. Some donned gloves without properly washing hands. Some washed their hands for less time than required. Items were stored in a hand sink basin.

Osaka Japanese Restaurant

7265 Martin Way E, Olympia

Feb. 2: 5 red; 5 blue.

Comments: Items on the menu were not properly marked as raw or cooked to order. The hand sink lacked splash guards.

Buddies Grocery

6501 Martin Way E., Olympia

Grocery

Feb. 2: 0 red; 2 blue.

Comments: Some products were stored on the floor rather than at least 6 inches above the ground.

Deli

Feb. 5 red; 10 blue.

Comments: There was no food thermometer. There were improper protections from potential food contamination. The sink lacked splash guards and food was prepped near the sink. Test strips for the sanitizer were absent.

Chipotle Mexican Grill

1177 Marvin Road NE, Lacey

Feb. 2: 25 red; 0 blue.

Comments: Food was improperly held under 135 degrees instead of at or above that temperature.

Iron Rabbit Restaurant and Bar

2103 Harrison Ave. NW, Olympia

Feb. 2: 0 red; 5 blue.

Comments: The sanitizer concentration levels were inadequate.

Outback Steakhouse Restaurant

2615 Capital Mall Drive SW, Olympia

Feb. 3: 0 red; 5 blue.

Comments: Mechanical warewashing machine did not reach 160 degrees as required.

Applebee’s

2500 Capital Mall Drive SW, Olympia

Feb. 4: 10 red; 0 blue.

Comments: Temperature controlled salsa was at 52-58 degrees rather than at or below 41 degrees.

Stack 571 Burger and Whiskey Bar

625 Black Lake Blvd. SW, Olympia

Feb. 4: 5 red; 5 blue.

Comments: Diced tomatoes were at 45 degrees rather than at or below 41 degrees as required. The dishwasher did not dispense sanitizer.
