Restaurant inspections for Feb. 23

Reading inspection scores:
- Attention should be given to the type of violation (red versus blue) and whether the restaurant has a history of red violations or repeat violations.
- Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
- Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
- Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Feb. 7: 85 red; 3 blue.
Comments: Food worker cards were unavailable upon request. Soups, sauces, meats were improperly cooled. Sauce in a pan was improperly held under 135 degrees rather than above that temperature. TCS foods were improperly held above 41 degrees rather than below that temperature. Thermometers were absent or not used. Some items on the menu were not marked cooked to order as required. Chemicals in a spray bottle were not labeled. Ice scoops at the soda and ice machines were stored in ice with handles entering the ice.

Food Service Establishment permit
Feb. 7: 25 red; 5 blue.
Comments: TCS foods were improperly held below 135 degrees rather than above that temperature. Sanitizer was not made at the time of inspection.
Grocery Stores and Confectioneries permit
Feb. 7: 15 red; 2 blue.
Comments: TCS foods were improperly held above 41 degrees rather than below that temperature. The inspector found inaccurate thermometers. Food was found stored on the freezer floor rather than at least 6 inches above the floor.
Meconi’s Italian Subs
2527 Marvin Road NE, Lacey
Feb. 7: 45 red; 3 blue.
Comments: Employees washed hands for less than the required 20 seconds. TCS foods in multiple cold units were held above 41 degrees rather than below that temperature. The units had inaccurate thermometers. Thermometers were not used to measure internal food temperatures. In-use utensils were improperly stored in standing water at 50 degrees rather than below 41 degrees or above 135 degrees.
Fujiyama Japanese Steakhouse
2930 Capital Mall Drive, Olympia
Feb. 7: 35 red; 8 blue.
Comments: No food worker cards were available during the inspection. Three containers of fully cooked and cooled rice were found at room temperature. Staff did not have access to a thermometer. No sanitizer chemical was available for cleaning cloths. The inspector observed shrimp improperly thawing in a sink.

Comments: Chicken was found cooling in pan greater than 2 inches in depth rather than below that depth.

Feb. 8: 25 red; 0 blue.
Comments: A corn dog was improperly held at 122 degrees rather than above 135 degrees as required.
Chevron
1601 Marvin Road NE, Lacey
Feb. 9: 35 red; 5 blue.
Comments: An employee did not wash their hands between tasks or wear gloves as required. A hand washing sink was not operational. Sanitizer was not ready at time of inspection.
Bacco Trattoria Italiana
9321 Martin Way E.
Feb. 9: 75 red; 5 blue.
Comments: Food worker cards were not available upon request. An employee washed their hands for less than the required 20 seconds. In a repeated violation, the kitchen hand sink did not have soap and the scullery hand sink was not operational. Shell stick tags were not completed as required. In another repeated violation, TCS foods such as rice, lasagna and meat sauce were found improperly cooling. Unit thermometer was not easily available. Sanitizer was not ready at the time of inspection. A re-inspection will be required.

Comments: Meats were found above 41 degrees rather than below that temperature as required.

Feb. 10: 10 red; 5 blue.
Comments: Cheese was improperly held at 50 degrees rather than at or below 41 degrees. The warewashing facility was out of sanitizer.
Brewery City Pizza
2705 Limited Lane NW, Olympia
Feb. 14: 15 red; 6 blue.
Comments: Lettuce, tomato and the ambient temperature of the salad preparation table were above 41 degrees rather than below that temperature. The inspector did not find a thermometer inside the salad preparation table. Chicken was observed thawing at room temperature. A knife was being improperly stored in a sanitizer bucket between uses.
Evergreen Valley Espresso

Feb. 15: 5 red; 5 blue.
Comments: Food worker cards were not available during the inspection. There were no test strips available for the sanitizer.
The Nom Nom Stop

Feb. 15: 10 red; 0 blue.
Comments: Items in the display cold hold were 47 degrees rather than at or below 41 degrees.
Teriyaki Delight
855 Trosper Road SW, Tumwater
Feb. 16: 10 red; 13 blue.
Comments: Small cans of chemicals were stored over the food storage area. Chicken was observed thawing at room temperature.
Pho 102
5765 Littlerock Road SW, Tumwater
Feb. 18: 40 red; 13 blue.
Comments: The person in charge failed to identify and correct red violations and must complete certified training. Noodles were observed cooling in a bowl rather than in a 2-inch pan. Sprouts were observed at 56 degrees rather than at or below 41 degrees. The warewashing sanitizer was not strong enough. In a repeated violation, the rice paddle was stored in room temperature water. In another repeated violation, there were no chlorine bleach sanitizer test kits available.
Sushi Kyo
1908 Fourth Ave. E., Olympia
Feb. 18: 40 red; 8 blue.
Comments: The establishment did not properly document parasite destruction for sushi grade fish. In a repeated violation, time as control was not documented for sushi rice. The temperature in a home-style cold hold was between 42-23 degrees rather than at or below 41 degrees. A thermometer in an open-air cold hold was broken. A home-style cold hold unit was used for TCS foods rather than a commercial unit. A vacuum sealed fish was found thawing while still sealed.
