Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.

Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.

Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.

If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.

If red points exceed 100 and there’s a lack of managerial control, the county will shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.

Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Comments:

  • A person-in-charge was absent during hours of operation and food preparation.
  • Food worker cards were unavailable upon request.
  • TCS foods in hot holding measured below 135 degrees. Hot held TCS foods must at or above 135 degrees.
  • A speed rack of foods had incorrect time labels. There was no written policy for using time as a public health control.
  • TCS foods in cold holding measured greater than 41 degrees. Cold held TCS foods must be at or below 41 degrees.
  • An ice machine was dirty with a mold-like substance.
  • Utensils were stored in room temperature water. In-use utensils must be stored in hot or cold water or on a clean surface.

Garcon de Donburi

Oct. 22: 55 red points; 6 blue points.

Comments:

  • The person-in-charge did not demonstrate active managerial control.
  • A food worker rinsed their hands in a prep sink prior to putting on gloves. Workers must wash their hands in a dedicated hand wash sink before putting on gloves and after handling raw meat, chemicals or otherwise contaminating their hands.
  • Multiple hand-wash sinks were blocked, not fully stocked or lacked hot water.
  • A cutting board used for raw meat was rinsed in a prep sink and air dried. Food contact surfaces must be washed, rinsed and sanitized after contamination and with a prescribed frequency.
  • In-use utensils were stored in room temperature water. In-use utensils must be stored in hot or cold water or on a clean surface.
  • Multiple non-food-contact-surfaces were dirty and contaminated.

The Lemongrass Café

Oct. 28: 55 red points; 3 blue points.

Comments:

  • A food worker handled a raw animal product and did not remove their gloves and wash their hands prior to working with ready-to-eat foods.
  • Produce was washed after being cut rather than before being cut.
  • Tongs used for raw animal products were stored in the same container as tongs for cooked products. A food worker used tongs interchangeably for raw animal and cooked products.
  • Raw, whole-shell eggs were stored above ready-to-eat foods rather than below and away.
  • One restroom hand sink did not have warm water that reached 100 degrees.

Oct. 22: 25 red points; 12 blue points.

Comments:

  • The person-in-charge was not knowledgeable about food safety temperatures or actively managing risks.
  • TCS foods in an open display measured more than 41 degrees. Cold held TCS foods must be at or below 41 degrees.
  • Food was stored next to car wash chains.
  • An ice machine had a moldlike substance.
  • Non-food-contact surfaces were improperly constructed. They were not smooth, non-absorbent or easily cleanable.
  • Shelving needed to be at least 6 inches above the floor.

Puerto Vallarta Restaurant

1400 Galaxy Drive NE, Lacey

Oct. 22: 35 red points; 0 blue points.

Comments:

  • Shredded chicken, rice and mashed beans in a steam table measured 128-134 degrees. Hot held TCS foods must at or above 135 degrees.
  • Sliced tomatoes, shredded lettuce and enchiladas in a prep fridge measured 42-43 degrees. Cold held TCS foods must be at or below 41 degrees.

La Patrona

5409 Capitol Blvd. SE, Tumwater

Oct. 28: 25 red points; 5 blue points.

Comments:

  • Diced tomatoes, cut lettuce and other TCS foods were missing date marks. Date marks are required for ready-to-eat TCS foods which must be used or discarded within seven days of opening or preparing.
  • Raw whole-shell eggs were stored above ready-to-eat foods in a two-door reach-in and produce walk-in. Raw animal products must be stored below and away from ready-to-eat foods.
  • Salsa on ice, sliced tomato in a prep unit, meat and cooked vegetables in an under counter cold hold all measured more than 41 degrees.
  • Produce from a delivery and in a walk-in were stored on the ground. Foods in a freezer were stored on the ground. Foods must be at least 6 inches above the ground.

Charley’s Grilled Subs at Capital Mall

625 Black Lake Blvd SW, Olympia.

Oct. 25: 25 red points; 0 blue points.

Comments:

  • Cooked boneless chicken wings measured 90-120 degrees and non-crispy cooked bacon measured 70-75 degrees. TCS foods must be kept cold or hot rather than at room temperature.

Oct. 24: 15 red points; 10 blue points.

Comments:

  • Cold-held shredded lettuce measured 47 degrees. Imitation crab measured 51 degrees. Cold held TCS foods must be at or below 41 degrees.
  • A thermometer was not available during the inspection.
  • Containers of cooked noodles were not date marked.
  • An in-use chlorine solution was too weak as prepared.
