Left Bank Pastry gets 50 red pts. Here are Thurston food safety inspections for Dec. 11

Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.
  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
  • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.
  • If red points exceed 100 and there’s a lack of managerial control, the county will shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.
  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Comments:

  • Cooked onions and apples were observed cooling while covered and at greater than 2-inch depths. TCS foods must be cooled uncovered, at 2-inch depths or less until they reach 41 degrees or below.
  • Eggs and quiches were stored at room temperature. TCS foods must be kept hot at or above 135 degrees or cold at or below 41 degrees.
  • A deli-slicer had visible build up dried onto it. The slicer must be washed, rinsed and sanitized every four hours or between uses.

Evergreen Gyros

2411 Harrison Ave. NW, Olympia

Dec. 2: 35 red points; 13 blue points.

Comments:

  • No certified food protection manager was on staff. All food establishments must have at least one person with a certified food protection manager certificate.
  • Tomatoes were not washed prior to preparation. All raw fruits and vegetables must be thoroughly rinsed under running water before being cut, used, cooked or served.
  • Various TCS foods measured greater than 41 degrees in a two-door standing cold hold unit.
  • The establishment did not comply with their permit and operating procedures. Food flow diagrams were not updated for the latest food menu and some food items were obtained from an unapproved source.
  • A sanitizer solution was too weak as prepared.
  • Sanitizer test strips were unavailable.
  • A rice utensil was not properly stored or replaced every four hours at room temperature.

Thai Pavillion

303 Fourth Ave. E, Olympia

Dec. 2: 35 red points; 7 blue points.

Comments:

  • The person-in-charge was not present. The inspector did not observe active managerial control and staff could not identify a person in charge when asked.
  • Shrimp was stored next to pork and adjacent to raw eggs. Different types of raw animal products must be stored separately.
  • Rice was improperly cooled and reheated after overnight storage in bins deeper than 2 inches.
  • Food contact surfaces were improperly cleaned and maintained. A vegetable slicer had visible debris and an unclean appearance.
  • The physical facility was not properly cleaned and maintained. The inspector instructed the establishment to ensure all garbage and food debris be removed, food storage areas are cleaned and ceiling tiles are replaced.

Toto’s Wok Teriyaki

Comments:

  • Non-continuously cooked chicken was stored in steam tables without a written procedure in place.
  • Cooked chicken was stored in cardboard containers rather than plastic or stainless-steel containers.
