Japanese eatery scores 75 red points. Here are Thurston food inspections for Jan. 15

Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.
  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
  • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.
  • If red points exceed 100 and there’s a lack of managerial control, the county will shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.
  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Fujiyama Japanese Steak House and Bar

Jan. 8: 75 red points; 3 blue points.

Comments:

  • An employee handled raw meat with gloved hands and changed tasks without removing their gloves and washing their hands.
  • One handwashing sink lacked paper towels and another sink in the kitchen was blocked by a trash can. A server station hand sink was blocked by a rack.
  • Raw shell eggs were stored above ready-to-eat vegetables in a prep unit. Raw animal proteins must be stored below or away from ready-to-eat foods and separated by species.
  • Cooked and diced chicken measured 52-55 degrees in room temperature storage. TCS foods must be kept under temperature control, either at or below 41 degrees or at or above 135 degrees.
  • A salad mix at a server station merchandiser measured 45-48 degrees. Yum Yum sauce on ice measured 55 degrees. Cold held TCS foods must be at or below 41 degrees.
  • Calamari was observed thawing on a counter. TCS foods must be thawed under refrigeration or under cold running water.

Jan. 9: 45 red points; 0 blue points.

Comments:

  • The person-in-charge was unable to relay the proper sanitizing procedure for food contact surfaces.
  • A food worker card was missing. All employees handling ready-to-eat foods and equipment must have a valid Washington state food worker card.
  • All food establishments must have an ill food worker policy. The establishment was directed to develop and adhere to a policy within two weeks.
  • The establishment operated without meeting conditional requirements following a change of ownership. The conditions include installing a three-compartment sink for wash and addressing a water leak.

Jan. 6: 30 red points; 8 blue points.

Comments:

  • No person-in-charge was present and no certified food manager was on staff. Employees were unable to demonstrate food safety knowledge.
  • Boba pearls were kept at room temperature with inadequate logging of time limit.
  • No sanitizer solution buckets were made at the time of inspection.
  • In-use utensils were improperly stored. Waffle batter with egg utensils were stored in room temperature water and changed daily. In-use utensils must be kept under temperature control or changed every four hours.

Jan. 8: 20 red points; 0 blue points.

Comments:

  • The person-in-charge demonstrated limited food safety knowledge.
  • The hand-washing facilities lacked drying provisions and a trash can.
  • Imitation crab and salmon in a sushi cooler measured 45 degrees. Cold held TCS foods must be at or below 41 degrees.

Jan. 8: 20 red points; 0 blue points.

Comments:

  • The hand-washing sink near a serving window was not stocked with soap or paper towels.
  • Potato salad at a salad bar measured 46 degrees above the ice line. Cold held TCS foods must be at or below 41 degrees.

Jan. 7: 15 red points; 0 blue points.

Comments:

  • The certified food manager failed to maintain product safety.
  • A half-size pan full of burritos measured 69 degrees. The food was received at 10:30 a.m. in a hot temperature condition and should have been hot held at 135 degrees.

Comments:

  • The person in charge was unaware of the establishment’s risk category and approved operational plans. They also were unaware of proper utensil equipment cleaning requirements.
  • The establishment operated outside their permitted risk category. Their permit did not allow for hot holding food.

Comments:

  • Pre-packaged burritos and bagel sandwiches in a two-door reach-in cold-hold measured 44-48 degrees. Cold held TCS foods must be at or below 41 degrees.

700 Lilly Road NE, Olympia

Jan. 8: 10 red points; 0 blue points.

Comments:

  • The establishment reported workers washed produce, cooled TCS foods and assembled sandwiches. The facility lacked a prep sink for produce washing. The establishment’s medium risk permit does not allow for cooling. The original approval letter stated food would be produced and packaged at the owner’s other permitted food establishment.

Comments:

  • A food worker was missing a current card. All food workers must have current, valid cards at all times.
  • An in-use sanitizer was too weak as prepared.
  • Non-food contact surfaces were not cleaned or maintained.
