Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.
  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
  • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.
  • If red points exceed 100 and there’s a lack of managerial control, the county may shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.
  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Comments:

  • The person in charge was unable to maintain active managerial control of uncontrolled risk factors for food safety.
  • Raw chicken was in a walk-in cold hold above ready-to-eat sauces. Eggs also were stored above ready-to-eat foods. All raw animal products must be stored below and away from ready-to-eat foods.
  • Rice noodles and portioned potatoes were observed cooling in a walk-in cold hold at a depth greater than 2 inches. TCS foods must be cooled uncovered in 2-inch-deep pans or less until they reach 41 degrees.
  • Workers did not temperature control or use time-as-a-public-health-control for boba tapioca pearls and cut cabbage. Both those items are considered TCS foods.
  • No thin-tip thermometer was available during the inspection.
  • A ware-washing machine did not have a chemical sanitizer.
  • Food workers washed dishes in a prep sink. Food establishments must have designated food prep sinks that are not used for hand-washing.

Comments:

  • The person in charge was not performing duties to maintain active managerial control of risks.
  • Only an expired food worker card was available during the inspection.
  • Restroom hot water measured less than 100 degrees.
  • Hot-held cooked meats and french fries measured cooler than 135 degrees. Hot-held TCS foods must measure at or above 135 degrees.
  • Cold-held raw meats and fresh salsa measured warmer than 41 degrees. Cold held TCS foods must measure at or below 41 degrees.
  • A chlorine sanitizer solution in a bucket was undetectable.
  • A toilet facility was unavailable for seated customers.

Comments:

  • Active managerial control was not demonstrated and there was no certified food protection manager on site.
  • There were no current food worker cards on site.
  • Time-as-a-public-health-control procedures were not properly followed. Foods utilizing these procedures must be discarded after four hours with proper documentation.
  • There was evidence of pests throughout the facility.
  • No probe thermometer was available to monitor food temperatures.
  • An in-use sanitizer was not available during the inspection.

March 26: 35 red points; 8 blue points.

Comments:

  • Wrapped cooked tofu was observed cooling in a prep unit and cooked rice noodles were observed cooling while covered. TCS foods must be cooled uncovered at a 2-inch depth until the foods reach 41 degrees.
  • Various TCS food items measured warmer than 41 degrees in a prep unit. Cold held TCS food items must be at or below 41 degrees.
  • Wiping cloths were not properly stored in a sanitizing bucket.
  • In-use utensils were improperly stored at room temperature.

Comments:

  • An employee had an expired food worker card.
  • A hand-washing sink was not used only for hand-washing.
  • A chlorine sanitizer solution was too strong as prepared.
  • Light was visible through various spots along the door frame. Entrances must be controlled to prevent potential pest entry.
  • The facility was cited for potential food contamination by miscellaneous sources.
  • A three-compartment sink for ware-washing was broken. Chlorine test strips were unavailable.

Comments:

  • Hot-held hot dogs measured 110 degrees. Hot-held TCS foods must be at or above 135 degrees.
