Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.

  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.

  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.

  • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.

  • If red points exceed 100 and there’s a lack of managerial control, the county may shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.

  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Comments:

  • The person-in-charge did not have a certified food protection manager certificate.
  • A hand wash sink near a prep area was blocked by a trash can and there were containers in the sink basin. Hand wash sinks must be readily available and adequately stocked.
  • Chicken was found cooling on ice on a counter in depths greater than 2 inches. Broth was observed cooling in a walk-in cold hold while on ice at depths greater than 2 inches. Employees were not checking temperatures with a thermometer. TCS foods must be cooled uncovered, at depths of 2 inches or less, in a refrigerator until it reaches at or below 41 degrees within four hours.
  • Tofu measured 50 degrees while cold held and a 2-door cold hold unit measured 45-53 degrees. All cold-held TCS foods must be kept at or below 41 degrees.
  • Cooked garlic in oil was kept at room temperature at 60-63 degrees. TCS foods require temperature or time control.
  • A dishwasher sanitizing solution was inadequately prepared.
  • Food contact surfaces were improperly washed, rinsed and sanitized. Deli slicers had visible debris.

Comments:

  • The certified food protection managers failed to monitor and correct improper food handling procedures being carried out by workers.
  • There was very little to no hot water flow at a hand-wash sink in a kitchen.
  • In-use supplies of fried, breaded chicken pieces in a two-door prep fridge measured 43-45 degrees. These items were not thoroughly cooled to 41 degrees.
  • Bean sprouts were on ice at 39-45 degrees rather than being cold held at or below 41 degrees as required.
  • Wiping towels for contact sanitizing did not have the proper sanitizer strength.
  • A dishwasher sanitizer solution was not strong enough.

Comments:

  • Certified food managers did not ensure safe food handling practices were monitored.
  • The inspector observed seven pans of cooled chicken in a walk-in cooler from the previous night measured 47 degrees. Seven-gallon bags of mashed potatoes from the previous night measured 47 degrees. These must have been cooled uncovered in less than 2-inch depths until they reached 41 degrees. Cold-held TCS foods must be kept at or below 41 degrees.
  • There was a large gap at a door threshold of a building with freezers and other dry storage.
  • TCS foods in a walk-in cooler measured 45-47 degrees. Cold held TCS foods must be at or below 41 degrees.
  • An in-use sanitizer solution was too weak.

Comments:

  • Multiple food worker cards were expired.
  • Items in the back cold hold measured 45 degrees. Cold TCS foods must be kept at or below 41 degrees.
  • A back cold-hold unit had broken thermometers. All cold hold units must have accurate, easily readable thermometers in them.
  • Workers washed lemons in a three-compartment sink without an approved plan. Produce must be washed in a prep sink or with an approved plan.
  • There was a black mold-like substance in an ice maker. Food contact surfaces must be kept clean and free of buildup.
