Reading inspection scores:

Attention should be given to the type of violation (red versus blue) and whether the restaurant has a history of red violations or repeat violations.

Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.

Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.

Comments: Sanitizer at 50 ppm chlorine. REPEAT: Hand wash sink observed with multiple blenders in basin. Hand wash sinks must ONLY be used for hand washing. Any other use will result in cross contamination. Corrected: Blender removed. Ice scoop stored directly in ice with handle contacting ice. In-use utensils must be stored in a way that prevents cross contamination. Corrected: Scoop moved to hanger on side. Chlorine test strips not available. Sanitizer test strips must be available and used to determine adequate sanitizer concentration. Correction: Obtain and use.

Comments: Contact sanitizer, DW machine did not provide any chemical sanitizer at final rinse. Use test paper to monitor proper sanitizer strength (50 ppm chlorine). Supply bottle was empty. Corrective action: Bottles changed. One employee food worker card expired. Obtain renewed card in two weeks.

Comments: Ware wash at 100 ppm chlorine, sani bucket at 200 ppm quat. Eight food worker cards missing, one expired. All food workers must obtain valid cards within two weeks of hire. All cards must be on site and available for inspection. Correction: Obtain within two weeks.

Comments: Sani-bucket/spray at 300 ppm quat. Five food worker cards missing. All food workers must obtain valid cards within two weeks of hire. Correction: Obtain within two weeks. Back hand wash sink lacks paper towels. All hand wash sinks must be stocked with soap, warm water and paper towels. Corrected: Paper towels provided during inspection. Ensure all employees wash hands for full 20 seconds after using POS (and returning to food handling).

Comments: Hand sink blocked, used as a table to support plates. Need to keep hand sink free/available at all times. Hot water only at 75 F. Must be 100 F minimum. Provide greater distance from pizza on hot tray to employees contact with apron. Toss expired pizzas in trash once removed.

Comments: One employee food worker card missing. All employees must obtain valid food worker card within two weeks of hire. Correction: Obtain within two weeks. Raw bacon stored above ready-to-eat cheese in walk-in. Raw animal proteins must be stored below and away from ready-to-eat foods. Correction: Bacon moved. Dunnage rack in freezer replaced. Work orders are in for missing tiles and decaying wall behind ice machine. This area is of high priority for repair and must be repaired by next inspection. Inspection conducted in response to multiple complaints, addressed both with PIC. All hot hold temperatures verified and time as a control system being used correctly. Good hand washing and no bare hand contact observed. Ensure all foods in time as a control rack are assigned proper pull times. Cook temperatures also verified and adequate during inspection.

Comments: All freezers, frozen. One food worker card expired. All food workers must obtain a valid food worker card within two weeks of hire. Correction: Obtain within two weeks.

Comments: All freezers, frozen. Sani-bucket at 200 ppm quat. Quiches and sandwiches in front glass make case at 47 and 46 F. Potentially hazardous foods must be held at 41 F and below. Corrected: Case temperature dial bumped during cleaning this afternoon, all potentially hazardous foods moved to walk-in. Foods temped this morning at 38-39 F (per PIC). Case re-adjusted, monitor temperatures and only refill after adequate temperature reached.

Comments: Dirty or used utensils placed in hand sink. Keep hand sink clear at all times. Wash and sanitize food related equipment in three-compartment sink ONLY. Bowl of cooked tapioca pearls placed in three-compartment sink. Use food prep sink ONLY for food. No sanitizer prepared or used when washing pitcher, equipment ware. Must sanitize these items every time. PIC operator has tobacco and cigarette-making equipment with apparent in-use state in kitchen. Remove and keep these products out of kitchen. Food cans must not be used for food storage once they are opened. Transfer food to food grade, sealable container.

Comments: Contact sanitizer at 300 ppm quat. Two bowls of rice noodles cooling at 3-4 inches deep at 68-75 F. Bowls also covered. Broth cooling in buckets at 180 F in walk-in cooler. These foods must be cooled at 2 inches of food depth and uncovered. Frozen ice wands were not utilized. Dish wash machine does not provide any sanitizer (chlorine) in final rinse. Chemical supply container is empty. Replace or manually sanitizer dishes and equipment in three-compartment sink.
