Reading inspection scores:
- Attention should be given to the type of violation (red versus blue) and whether the restaurant has a history of red violations or repeat violations.
- Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
- Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
- Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Comments: The grab and go quiches and bagels weren’t properly labeled.
Bayview Thriftway Market Place
516 Fourth Ave. W., Olympia
July 27: 60 red; 3 blue.
Comments: The chicken in the hot hold area was 100 degrees rather than at or above 135 degrees as required. The salad bar was 47 degrees rather than at or below 41 degrees as required. Fans in the walk-in cold hold were contaminated and needed to be cleaned. All these items were corrected and addressed by a later inspection on Aug. 8.
Rush in Alaskan Dumplings LLC
205 Fourth Ave. E., Olympia
Aug. 1: 10 red; 5 blue.
Comments: The preparatory cold hold area was 45 degrees rather than 41 degrees or lower. The thermometer in the preparatory cold hold read 60 degrees when the actual temperature was 45 degrees. The in-use cleaning sanitizer was improperly labeled.

Comments: The under-counter cold hold lacked a thermometer. The drying rack and cupboard door was contaminated and in disrepair.

Comments: There were issues with maintaining proper cold holding temperatures as well as maintaining and cleaning surfaces that don’t contact food. No specifics on these two deficiencies were noted.

Comments: The hand wash sink was full of dishes when the inspector walked in. A little wastewater had spilled outside near a drain.

Comments: The inspector noted TCS foods were improperly cooled and stored in cold holding. The TCS foods were held at above 41 degrees.

Comments: Grab n Go items were not properly labeled in a cold hold area. The in-use cleaning sanitizer was not strong enough.
Chambers Prairie Market Shell / Dask LLC, Deli

Aug. 29: 45 red; 10 blue.
Comments: An employee did not have their food worker card during inspection. The produce sink was being used as a mop sink. Raw bacon and chicken were found stored above ready to eat foods rather than below and away. A pizza in a hot case was at 122-125 degrees rather than 135 degrees or above. Paper towers were stored directly above clean utensils with no splash guard between them. There were boxes of food stored on the floor rather than at least 6 inches off the ground. The inspector labeled the establishment “medium risk.”

Comments: There were issues with toxic substances being properly identified, stored or used. There was also a deficiency related to adequate equipment for temperature control. The inspection notes do no share specifics about those two deficiencies.

Sept. 19: 35 red; 0 blue.
Comments: Dough was held at above 41 degrees in several units rather than at or below that temperature. An employee didn’t wash their hands after handling trash and before placing new gloves on.

Comments: The inspector noted there was no room temperature storage as well as issues with wiping cloths being properly stored and used. In-use utensils were not properly stored, and physical facilities were not properly installed, maintained or cleaned. The inspection notes do no share specifics about those three deficiencies.
