Lacey Thai spot hit with 55 red points. Here are Thurston food safety scores for Oct. 1

Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.
  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
  • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.
  • If red points exceed 100 and there’s a lack of managerial control, the county may shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.
  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Sept. 23: 55 red points; 11 blue points.

Comments:

  • No active managerial control with multiple repeat red violations. There must be a trained person-in-charge at the facility at all times, practicing active managerial control.
  • More than three eggs were pooled in the prep cold hold and at room temperature. Except for immediate service, no more than three eggs shall be pooled together.
  • Rehydrated noodles and pooled eggs were at room temperature. TCS foods must be kept at or below 41 degrees or 135 degrees or above.
  • Half and half in the front glass was cold held at 68 degrees. TCS cold held food must be kept at or below 41 degrees.
  • Shrimp was thawed at room temperature in the prep sink. Foods must be thawed under refrigeration or cold running water.
  • Food equipment was stored in the employee restroom. Equipment must be stored to prevent contamination.
  • The back hand sink was leaking and was slow to drain.

Sept. 24: 40 red points; 3 blue points.

Comments:

  • A large bowl of raw chicken in the walk-in was observed over raw shelled eggs directly above bags of apples and boba tapioca pearls. All raw meats must be stored in a way to prevent cross contamination.
  • Multiple TCS foods were observed at room temperature storage. All TCS foods must be held under proper time or temperature requirements. Hot foods must be kept at 135 degrees or above, and cold foods must be kept at 41 degrees or below. Cooling at room temp is not permitted.
  • Multiple foods in the prep unit were above 41 degrees. All foods in cold hold must be kept at 41 degrees or below at all times.
  • Shrimp was observed thawing at room temperature in a large bucket on the ground. Proper thawing must happen under refrigeration or constant running water.

Johnson’s Smokehouse & Eatery

8312 Diagonal Road Southeast, Olympia

Food Service Establishment

Sept. 25: 20 red points; 0 blue points.

Comments:

  • One Food Worker Card was from an unapproved provider. All food workers must have a valid Washington Food Worker Card.
  • Cooked sausages in hot hold were at 131 to 141 degrees. TCS foods must be hot held at 135 degrees or above.
  • Some vacuum sealed foods were missing proper labeling or were not kept in accordance with approved plans.
  • The inspection noted a CFPM certificate is required prior to the next routine inspection.

Comments:

  • No Certified Food Protection Manager certificate was available upon request. All food establishments must have at least one person on staff with CFPM certification.
  • Noncompliance with permit.
  • Updated written procedures for using time as a control for boba pearls was unavailable.
  • The three compartment sink, prep sink and ice machine directly drained at the time of inspection. They must be indirectly drained to prevent contamination from back up.

Sept. 23: 15 red points; 0 blue points.

Comments:

  • No CFPM was available as required at the last routine inspection. This is required and must be obtained within 60 days.
  • The handwashing sink had a hose attached to the water valve and had no mixing valve. Handwashing sinks must have hot and cold water from a mixing valve and must remain clear of objects that discourage handwashing.

Comments:

  • A vomit and diarrhea plan was not completed and a kit was missing at the time of inspection. All facilities must have a completed vomit and diarrhea clean up plan and kit available.
  • Previously cooled TCS foods were reheating in a hot hold that was turned all the way up. Employees must ensure previously cooled foods are rapidly reheated to 165 degrees within two hours.

Comments:
