Olympia taco truck hit with 50 points in food inspection scores for April 15

Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website. Reading inspection scores:

  • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.
  • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
  • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
  • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.
  • If red points exceed 100 and there’s a lack of managerial control, the county may shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.
  • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

Comments:

  • Three items were found at 60 degrees to 67 degrees and were being improperly cooled.
  • An employee did not wash their hands after handling raw fish and changing their gloves. Employees must wash their hands between glove changes and handling raw meats and seafood.

April 9: 40 red points; 10 blue points.

Comments:

  • The establishment had repeated red violations for toxic substances of in-use sanitizer that measured over 200 ppm. In-use chlorine sanitizer should measure between 100 and 150 ppm.
  • There was no date marking on a number of TCS, ready to eat and dairy products.
  • Half and half being cold held was at 43 degrees. All TCS foods in cold hold must be at 41 degrees or below.
  • A pet dog was in the seating/bar area. Animals are not allowed on premises of a food establishment, and the ‘Pet dogs not allowed’ sign needed to be moved forward.
  • There was no written vomit and diarrhea plan. Food establishments must have required written procedures available and present.
  • A food bucket of whipped margarine in the cold hold unit had a mold-like buildup inside.

Comments:

  • The establishment had repeated red violations of its handwashing station. The handwashing sink was used as a dump sink.
  • Raw tuna was observed being stored above RTE cabbage in a prep unit. Foods must be protected from cross contamination, and raw food must be separated during storage from RTE foods like vegetables.
  • Pico de gallo was observed cooling covered in amounts greater than 2 inches. TCS foods must be cooled in 2-inch amounts until 41 degrees or below.

Comments:

  • Hollandaise sauce made with eggs was stored on the counter at 83 degrees. TCS foods must be stored under time or temperature control for safety.
  • Dairy products were being stored between 44 degrees and 49 degrees. TCS foods must be cold held at 41 degrees or below.
  • The consumer advisory on menus has a disclosure but is missing the reminder statement.

Comments:

  • Cheese sauce in a bottle warmer was at 130 degrees. TCS foods must be held at 135 degrees or higher.
  • The Mobile Food Unit did not have hot water above 50 degrees during the inspection, which is considered an imminent health hazard. The facility closed while the hot water heater was turned up and reopened before the end of the inspection.
  • Wastewater was being disposed of improperly, and fresh water was not being filled according to approved plans.
  • The person in charge did not demonstrate adequate food safety knowledge during the inspection.

Comments:

  • The Certified Food Protection Manager certificate expired. It must be renewed by May 27.
  • A Food Worker Card expired.

Comments:

  • An employee donned a second glove to touch raw chorizo and then removed one to contact clean utensils without proper handwashing. Employees must not double glove and should remove their gloves and wash hands before utilizing utensils.
  • The in-use sanitizer measured at 0 ppm. The minimum is 100 ppm.

Comments:

  • Meatballs were placed in sealed ziplock bags, placed in the merchandiser and measured at 61 degrees. All cooled foods must be cooled in 2-inch shallow amounts, uncovered and unsealed or by using a tracking method to validate if it reaches the proper temperature.

Comments:

  • There were repeated violations of cold holding of several foods observed, with some items measuring above 41 degrees. All TCS food must be cold held at 41 degrees or below.
  • There were more than three expired FWCs.
  • The in-use sanitizer tested at 0 ppm, then it was remade and was too strong. The minimum is 100 ppm.

  • The ice machine was observed with buildup and mold-like debris.
