3 Thurston County health care workers disciplined

Disciplinary actions against health care providers in Thurston County were released June 10 by the state Department of Health.

Nurse Derek Paulsen was placed on probation for two years after Paulsen failed to contact a patient’s physician and the patient was found dead of cardiac arrest.

Beatrice Safford’s nursing license was reinstated in January after it was suspended in 2012 because she had not complied with a substance abuse program. Safford must continue to participate in a substance abuse program.

Rachel Leigh Meyers was granted a nurse credential in January, with the requirement that she continue a substance abuse program. Meyers was convicted of two felonies, four gross misdemeanors and three misdemeanors between 2008 and 2014.
