Reading inspection scores:

▪ Attention should be given to the type of violation (red versus blue) and whether the restaurant has a history of red violations or repeat violations.

▪ Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.

▪ Blue violations relate to overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.

Feb. 10: Routine check (125 red; 16 blue)

Comments: Red — Sushi worker was observed removing gloves bloody from raw shrimp, wiping hands on rag and apron, then touching serving tongs, refrigerator handles and food pans. Many foods on hot lines were double- or triple-panned with little or no water in between. Many foods were below minimum hot holding temperature. Cold serving lines were holding foods between 54-57 degrees. All cold holding/storage must be 41 degrees or colder. Handsinks were not accessible. Deep-fried items in merchandiser were improperly cooled. Blue — Accurate thermometers were unavailable. Improper thawing of raw chicken observed. Sanitizer was absent in food prep areas. Buffet utensil stored in standing water at 78 degrees. Store utensils dry and change every four hours, or store either above 135 degrees or below 41 degrees. Sanitizer test strips were unavailable. Reinspection — A reinspection was conducted Feb. 23, where there were 25 red violations noted and 16 blue violations.

Comments: Red — Some employees had expired foodhandler cards.

Feb. 26: Routine check (20 red; 0 blue)

Comments: Red — Broad rice noodles were not reheated fully to 165 degrees. Cook incorrectly responded to demonstration of knowledge query.

Mayan Family Mexican Restaurant

4820 Yelm Highway, Lacey

Feb. 25: Routine check (10 red; 15 blue)

Comments: Red — Shredded cheese in iced double container was at a temperature of 42-48 degrees. A smaller container is needed. Blue — Pitchers for drinking water being filled at handwash sink. Sanitizer wiping towels not being used on cutting boards and utensils. Single large knife stored uncleaned and in gap between equipment.

Comments: Red — Rice being cooled improperly.

Comments: Red — Wok full of cooked chicken at temperatures of 121-134 degrees. This must be hot held at 135 degrees or above. Person in charge could not answer knowledge questions correctly.
