Reading inspection scores:
▪ Attention should be given to the type of violation (red versus blue) and whether the restaurant has a history of red violations or repeat violations.
▪ Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
▪ Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.

May 9: Routine check (135 red; 21 blue)
Comments: Red — Person in charge was unable to demonstrate knowledge and unable to correctly answer questions regarding proper cooling procedure, cold holding temperatures, cooking temperatures, proper food storage and proper thawing procedures. Foodhandler cards were unavailable for two kitchen employees. Multiple employees were observed washing hands for less than 20 seconds, and some employees were observed not washing hands before placing gloves on. Employees were observed not changing gloves or washing hands between tasks. Handsink is inaccessible. Cabbage was not rinsed prior to prep. Raw meats were stored in walk-in cooler on racks above vegetables. Raw shrimp was being stored over pre-cooked foods, and raw chicken was being stored over vegetables. Raw eggs were being stored over vegetables and sauces. Pho broth was being cooled in five-gallon buckets on floor before being placed in walk-in cooler. Multiple items were being improperly cooled and stored. Rice in plastic tub was in depth greater than two inches, covered and being stored on counter at 51-62 degrees. Multiple items were in temperature danger zone during cold holding. Rice noodles in sushi display were at 56 degrees, rice in glass merchandiser was at 44 degrees, and rice noodle in prep table was at 46 degrees. Cold holding/storage must maintain temperature of 41 degrees or colder. Thermometers were not accurate. Stem thermometer read 42 degrees when placed on ice. Chemical bottles were unlabeled under three-compartment sink. Blue — Chicken was being thawed on floor in boxes. Gloves were being used improperly, increasing danger of contamination. Utensils were not being properly stored. Duct tape being used on food storage containers that leak or are damaged, on scissors and throughout facility. Hood system with excessive residue/buildup. Physical facility not properly maintained. Excessive residue and buildup throughout kitchen. The result of this inspection was to close this restaurant immediately for a 24-hour period. All employees were required to retake foodhandler class and pass the test. A reinspection is required. Reinspection: A reinspection was conducted and the restaurant was allowed to reopen on May 16.

Comments: Red — Two employees had expired foodhandler cards. Paper towels missing from wait staff handsink. Raw salmon and ahi tuna stored over ready-to-eat foods in sushi line prep cooler. Trays of raw shrimp near grill/wok were 67 degrees. Blue — Ice scoop placed on top of dusty ice machine. Inside of ice machine lid needs cleaning. Handles and doors of coolers/freezers in kitchen also need cleaning.

Comments: Blue — Large wire whisk stored uncleaned and on unclean ledge of ice machine. Large ice scoop stored in ice bin. Employee observed rinsing coffee brewer funnel basket in hand sink.
